We understand you are busy. If you forgot to re-enroll, you may simply use our online application to apply for the current year or call us to speak directly to a Care Navigator to walk you through the process.  


Re-enrollment is now closed

Approvals last for a full calendar year. Regardless of when you initially enrolled in a program, re-enrollment always occurs in the final quarter of a calendar year (the months of October, November, and December). Return to this page then for assistance with 2022 re-enrollment.
